beyfortus (RSV)
For infants born <35 weeks, CHD, or CLD, give a dose of Beyfortus soon prior to discharge. See screenshot below on ordering; it is currently available in Epic. For neonates < 5kg, the dose is 50 mg. For neonates >/= 5kg the dose is 100 mg.
We recommend continuing to administer Beyfortus on Tuesdays and Thursdays whenever possible for monitoring purposes. However, if the neonate happens to be discharged unexpectedly then please give prior to discharge regardless of day.
For infants born >/=35 weeks and <8 months of age (not meeting criteria above), will recommend vaccination with Beyfortus at the soonest outpatient follow up with the pediatrician. May use the dotphrase .nicubeyfortus in the dsicharge summayr instead of saying does not qualify for Synagis (that is our current default).
If there is concern the family may not be reliable for follow up and or a higher risk neonate that had a prolonged hospital course, may still administer per the Attending's discretion.
Please look closely for maternal vaccination with Abrysvo (32-35 weeks) and the timing of administration. If vaccination was at least 2 weeks prior to delivery, the neonate is covered during this season. If vaccination was <2 weeks prior to delivery, the neonate will require vaccination.
