HRIF Eligibility criteria:
Birth weight equal to or less than 1500 gm OR PMA less than 32 weeks gestation, OR
Birth weight greater than 1500 gm OR gestational age at birth equal to or greater than 32 weeks gestation, AND one of the following criteria was met during NICU stay:
Cardiorespiratory depression at birth (defined as pH <7.0 on an umbilical blood sample or a blood gas obtained within one hour of life) or an Apgar score of < 3 at five minutes or < 5 at ten minutes
Prolonged hypoxia, acidemia, hypoglycemia, and/or hypotension requiring pressor support
Persistent apnea which required medication (e.g., caffeine) for the treatment of apnea at discharge
Required oxygen for more than 28 days of hospital stay and had radiographic findings consistent with chronic lung disease
ECMO
Infants who received inhaled nitric oxide (iNO) for > 4 hours for PPHN
Infants with complex congenital heart disease
History of documented seizure activity
Evidence of intracranial pathology, including but not limited to, intracranial hemorrhage (grade II or worse), periventricular leukomalacia, cerebral thrombosis, cerebral infarction, developmental central nervous system (CNS) abnormality or “other CNS problems associated with adverse neurologic outcome.”
Complex congenital heart disease requiring surgery
Other problems that could result in a neurologic abnormality (e.g., history of CNS infection, documented sepsis, bilirubin above usual exchange transfusion level for gestational age, cardiovascular instability, hypoxic-ischemic encephalopathy, presence of hypotonia or hypertonia or asymmetry of tone at discharge, et cetera.)
Note that chromosomal abnormality and intrauterine drug exposure by themselves are not HRIF criteria.
